this form cannot be submitted on-line. Please print and complete the information
requested. Documentation must be submitted with your request i.e. Marriage
Certificate, Lease, Utility Bill or Closing Documents showing proof of
new address etc.
In-person: DO NOT sign the card until you are in front of a Peoples Credit
Union representative. Copies of your documentation will be made by a Member
Service Representative.
By-mail: Signatures on card must be notarized before you mail to the Credit
Union. Supporting documentation must accompany request.
Participating Company: Signature card must be signed in front of your
Company’s representative before submitting. A copy of supporting
documentation must be submitted with request.
